Our son is working in Belize for the year. He's holding a place at university for next year, but he's told us he's having doubts about the course. Have you any suggestions?

Sometimes people do re-examine their priorities during a gap year. Are his doubts about the course itself or the university? Whatever they are, he should talk them over with the university. They have made a commitment to each other. The university will have vast experience of students considering a change of direction. Remember, too, that he is thousands of miles from home; things may look very different once he is back.
